Bound Tree Medical (parent company to Sun Belt) will begin servicing all of your pharmaceutical needs starting March 1, 2009. Sun Belt has been a subsidiary of Bound Tree since they entered into a strategic agreement in 2002 and this change will enable us to more effectively service your account.

Beginning March 1, 2009, you will begin to place and receive your orders directly through Bound Tree. Depending on your location, your shipments will originate from one of three strategically located distributions centers: Southaven, Mississippi; Tempe, Arizona; or Visalia, California. For most customers, your delivery times will be notably reduced.

As a result of this change, all of the products that have been provided to you by Sun Belt will now be handled by Bound Tree. In addition to shipments, all order documentation will come directly from Bound Tree. We expect a seamless transition, but would like you to be aware of the following:

1. For Class II orders, you will no longer be using Sun Belt’s name and address on DEA Form 222.  Please refer to the Controlled Substance Ordering Guidelines to determine the Bound Tree Distribution Center closest to your location. You will use that address to complete and mail the DEA 222 forms.
2. Payments should be made out to Bound Tree Medical, LLC and submitted to the Bound Tree lockbox at:
